Under the supervision of the Occupation Injuries and Diseases Act (COIDA) of 1993 (amended in 1997), the funding that comes with the compensation act is fully administered and will only be distributed for individuals who are able to prove their eligibility and how much they deserve it. After downloading the form, be sure to fill it out completely with only accurate and correct personal details.
